SYMPTOMS
After two or more Fibre Channel adapters are installed but only one is connected to a single set of disks, no problems occur. However, after another set of disks is attached to the second adapter, you may receive Stop 0xA error message with the following parameters:
This error also occurs if any of the following devices from SCM are connected to the system:
1. Sandisk USB CF reader
2. Microtech USB Cameramate
3. Microtech ZIO-MMC (Multimedia card) reader
4. Zio-SD (Secure Digital) reader
5. USB-Smartmedia reader
CAUSE
This problem can occur after you apply the hotfix that is documented in the following Microsoft Knowledge Base article:

After the IRP is allocated, the thread information is not set.
RESOLUTION
To resolve this problem, obtain the latest service pack for Windows 2000. For additional information, click the following article number to view the article in the Microsoft Knowledge Base:

The English version of this fix should have the following file attributes or later:

STATUS
Microsoft has confirmed that this is a problem in the Microsoft products that are listed at the beginning of this article. This problem was first corrected in Windows 2000 Service Pack 2.
