4.00.1111 is the OEM Service Release 2 RTM build of Windows 95.

Prepare the hard disk with fdisk.exe, reboot, then format. format.com C:
Enter the WIN95 directory and launch setup.exe
Fill in required information then continue to the file copying process. During the last 10% of the copying, a missing file is needed. Just skip it.
Remove the floppy then reboot.
