SYMPTOMS
When you customize the Insert menu in Microsoft Office Word 2003, some of the custom menu items may be lost, rearranged, or repeated on the Insert menu.
CAUSE
This problem may occur when you add custom menu items to the Insert menu, and then you make changes to those custom menu items.
RESOLUTION
This problem was first fixed in a hotfix that is now contained in a service pack. If you installed the latest Microsoft Office 2003 service pack, you do not have to install the hotfix.

This problem is corrected in Microsoft Office 2003 Service Pack 1 (SP1).
