SYMPTOMS
When you open a custom Contact form, the File as format (on the General tab of the contact) that displays a combination of the FullName field and the Company field in the view may be shortened to Last, First name only instead of Company and Last name.
CAUSE
This problem occurs after you change the contact to use a custom Contact form.

The dates and times for these files are listed in coordinated universal time (UTC). When you view the file information, it is converted to local time.
