SYMPTOMS

If you unplug a USB audio device that is based on the port class system driver (PortCls.Sys) while the device is streaming, PortCls.Sys does not correctly handle the close notification (IRP_MJ_CLOSE) that occurs after the surprise removal notification (IRP_MN_SURPRISE_REMOVAL) is generated. This behavior prevents PortCls.Sys from correctly closing the stream and from performing the handle cleanup.

Additionally, the IRP_MN_REMOVE_DEVICE notification is not generated because open handles remain.
