In the Spell Dictionary command, you pressed Y after being prompted to create a new dictionary. Spell cannot create the dictionary because you gave an invalid filename for the dictionary or your disk is full.
Choose Dictionary again to enter a valid filename or quit Spell to make more room on your disk.
