SYMPTOMS
When you clear the Include Field Names check box for a form that returns a log in HTML format, no entries are saved when the form is submitted.
The following HTML formats do not log entries for the Save Results component of FrontPage:
HTML
HTML Definition List
HTML Bulleted List
CAUSE
The form fields are not converted properly to HTML paragraphs.
RESOLUTION
To resolve this problem, obtain Microsoft Office 2000 Service Release 1/1a (SR-1/SR-1a).

To temporarily work around this problem, select the Include Field Names check box for the form. To do this, follow these steps:
- Right-click the form, and then click Form Properties on the shortcut menu.
- In the Form Properties dialog box, click Options.
- In the Options for Saving Results of a Form dialog box, click the File Results tab.
- Select the Include field names check box, and then click OK to close all open dialog boxes.
STATUS
Microsoft has confirmed that this is a problem in the Microsoft products that are listed at the beginning of this article.
This problem was corrected in Microsoft Office 2000 SR-1/SR-1a.
