After killing explorer.exe and restarting it, the Start orb appeared.

Give any user no rights to acces the filesystem of c: in 2k3 and log in: you have no start button.

As for the Windows 95 Setup process images mentioned above, I actually think that they relate to open folders and applications within the Explorer shell, not Program Manager.
