SYMPTOMS

If you unplug a universal serial bus (USB) audio device during recording or playback, your computer restarts without warning.

CAUSE

This problem occurs because the Usbaudio.sys driver does not correctly manage I/O request packets (IRP) when you remove the USB audio device.
