SYMPTOMS
When you remove Plug and Play Universal Serial Bus (USB) device (a surprise removal) and an application fails to close the handle to that device, the application does not receive a subsequent device arrival notification when you plug the device in again.
In this scenario, the application may receive a device arrival notification for every other reinsertion of the device.
CAUSE
This issue can occur, because the HID Class driver does not handle surprise Plug and Play USB device removals correctly.
NOTE: This issue causes Windows to not notify the applciations that have registered to receive notifications for these devices.
RESOLUTION
To resolve this problem, obtain the latest service pack for Windows 2000. For additional information, click the following article number to view the article in the Microsoft Knowledge Base:

NOTE: To view the version, right-click the file in Windows Explorer, click Properties on the menu that appears, and then click the Version tab.
To work around this issue, close the device handle when the application receives a Device Removal notification.
STATUS
Microsoft has confirmed that this is a problem in the Microsoft products that are listed at the beginning of this article. This problem was first corrected in Windows 2000 Service Pack 1.
