SYMPTOMS
Any file extension, such as .htm or .stm (the default mapping for Ssinc.dll) can be mapped to Ssinc.dll through "App Mappings" in the Internet Service Manager. After mapping, the #include statement is used in the file (.htm or .stm for this case) to include other files. If a file, such as default.htm, is set as the default document and has such an #include statement in it and is then browsed as the default document (for example, http://server vs. http://server/default.htm), a 401 Access denied response is always sent to the browser (resulting in a password prompt) even if a valid username/password pair is entered.
RESOLUTION
