SUMMARY
In Word, to set a custom paper size for a document that you want to print to an ImageWriter, do the following:
From the Tools menu (the Edit menu in Word 4.0), choose Preferences.
Under Custom Paper Size (at the bottom of the dialog box), in the Width and Height boxes, type the width and height of your custom paper size. In Word 5.0, close the Preferences window; in Word 4.0, choose the OK button.
Note: Maximum page width is 22.75 inches, maximum page length is 22.75 inches
From the File menu, choose Page Setup.
Select Custom (with the new paper size), and choose the OK button.

In Word versions 3.x, the custom page setup option was located in the Page Setup dialog box. In Word versions 4.0 and 5.0 this option was moved out of the Page Setup dialog box so that this dialog box would more closely conform to the standard Apple Page Setup dialog box.
Note: Custom page sizes may be set only when the ImageWriter is selected in the Chooser as the current printer.
