I got the Blue Spine edition of IBM OS/2 Warp on CD-ROM off eBay and I'll image its floppies and CD.
But first, should I scan its outer box... and why?

If you plan on submitting it to the FTP, then yes, please scan the CDs, floppies and the box (make sure you follow the guidelines).

What exactly happened when "everything went wrong"? It's not difficult to scan the covers as lots of members already done that for other titles.

Then you need to properly press down on it by applying some weight etc. As long as the cover is flat against the scanner it should not be an issue.

It's very odd considering that the scanner doesn't do any focusing and as long as you have the cover flat against the screen it will scan it properly. Can you upload a sample?

Alright, just checked them.
Are these scans resized? They look extremely blurry and jagged for being 600dpi scans...
OS2 Box - Right Side: It's a bit blurry, can you put down some weight on it inside and retry? Same goes for OS2 Box - Top, but since it's a flap you can scan it with the flap open and put a book or something on top to press it down.
Does the Bonus Pak have a rear side? I only saw sides and front.
Otherwise it looks ok.
