Simple: if you don't trust it, don't use it...
When I look at the page and read some of the text I think it's not really from Twitter...
The fact that it's a survey and you MUST fill in your personal information... To me a survey is anonymous or at least possibly anonymous.
Second think I'm conserned about is the following:

A third think that rings some enormous alarm bells at me is THIS... Only one hit at Google and it's in fact your link, not even a hit at the website of Twitter itself... Even without the phrase survey it still has not many hits and still no real Twitter related.
P.S. could you copy the header of the e-mail? To see where it was send from... (edit your own information like name and e-mail address out of it before placing it!)

The header seems to be a header like every Twitter e-mail (ham-cannon.twitter.com for instance is the Twitter bulk mail server)... but i'm still confident that it's pure fishing.
The last link you gave is hosted at Google Docs... Twitter has it's own server(s)... and the fact that the link is shortend, why would you ever do that in an e-mail...
Another thing... at first it's called "Twitter Research Panel" and then "Twitter Research User Testing Panel"
All in all enough for me to delete the e-mail and/or make a note to Twitter...
